Former students who are no longer enrolled at UCSB may obtain official verification of enrollment and/or degree conferral by completing the Student Services Order Form and taking the following steps:

• Submit form to transcripts@sa.ucsb.edu
  • Use one form per order
  • Note the email or mailing address on the form
  • If requesting a letter, note the type of information to be included (or provide an example), the receiver’s name, and the deadline date, if applicable
• Submit payment.
  • Based on the type of order, the student services representative will provide a link to the appropriate payment form through Authorize.net.

Orders will not be completed without payment. Fees are non-refundable once orders are processed. Verifications will be delivered via email or standard US postal mailing. Overnight or international express services are not available.
