All forms must be signed and dated by the student.

All forms must be submitted by the student to the Office of the Registrar from their Umail account (current students). For DocuSign forms, students will be asked to provide a copy of their photo ID for authentication purposes.

Forms will not be accepted from a third party, such as a parent.

Forms are available using DocuSign, through the UCSB Student Success Hub, or to download as a PDF. PDFs require Acrobat Reader, a free download from Adobe.

Summer Cancellation/Complete Withdrawal

Summer Schedule Adjustment

Fees
Partial Fee Reduction
- Undergraduate students may use this form to request a 50% reduction in quarterly tuition, if all requirements are met. Available for Fall, Winter, and Spring quarters only.
 - Deadline: 15th day of instruction of the quarter (no exceptions).

Non-UC Sponsored Study Abroad
- Use this form to notify UCSB that you are leaving UCSB for one or more quarters to attend a study abroad program through another university.
 - The waiver of liability (PDF) must be submitted with the Leave Request form if the study abroad travel occurs in a country listed on the U.S. Department of State’s Travel Warning website.
 - Deadline: At least one week before departure date

Student Athlete Intent to Declare a Major
- NCAA student-athletes may use this form when they are prevented from officially changing or declaring a major and/or when the Director of Compliance needs a major verification immediately.
